      • The corners of the vinyl-covered foam play structures in the Infant Room were observed to be peeling and in disrepair, making them difficult to clean and sanitize.Please remove or replace the foam play structures with ones that have smooth, impervious, and easily cleanable surfaces.

      • 1) Push pins were used to hang decor in the toddler room.Please remove and replace the push pins with a safer alternative to prevent choking hazards and accidental punctures.2) The linoleum mats in the Preschool Room and the Before and After School Care Room, used underneath tables for easy cleaning, was observed to have rolled edges, creating a potential tripping hazard.Please replace the mats with ones that have flat edges.

      • 1) Child safety locks on the mini fridges in the Infant and Toddler rooms were observed to be unclipped.The operator was instructed to lock the refrigerators. Please ensure staff are reminded to lock the refrigerators after use to prevent children from accessing them.2) The handle for the valve beneath the handwashing sink at the daycare entrance was missing, creating a protrusion at children’s eye level.The operator created a temporary protective cover over the protrusion. Please implement a permanent repair to fully address the hazard.

      • 1) Several highchair straps in the Infant Room were observed to be frayed, compromising their integrity.Please identify and replace any frayed straps on the highchairs in the Infant Room.2) The grooves and crevices of the diaper changing table located in the Toddler Room was observed to be caked with dirt and debris.Please thoroughly clean and sanitize the entire surface of the diaper changing table, including all grooves and crevices.

      • Disinfectant was missing from the diaper changing station located in the Infant Room.The operator was instructed to make a bottle of chlorine solution measured at 100 ppm for the diaper changing station in the Infant Room.

      • The first aid kit located in the Toddler Room was missing a CPR mask.The operator promptly equipped the first aid kit with a CPR mask. Please ensure the first aid kit is fully stocked with the necessary supplies and that anti-septic wipes are within their expiry date.

      • The mini fridge located in the infant room was not operational, measuring at 23C.The operator was instructed to discard the leftover cut fruit stored in the cooler and to label the refrigerator to indicate that it is inoperable.

      • An expired bottle of Purexall Disinfectant 5 (2022) was used in the Pre K Room.The bottle was replaced with a chlorine solution spray bottle. Please inspect all Purexall bottles in the facility and discard any that are expired.

      • 1) Play mats in the Infant Room and the Before and After School room were observed to be peeling and in disrepair, making them difficult to clean and sanitize.Please remove and replace the play mats with ones that have smooth, impervious, and easily cleanable surfaces.2) Shelving units and play equipment throughout the facility were observed to be dusty and contain loose debris.Please remove all toys, supplies, bins, and equipment from the shelving units, and thoroughly clean and sanitize both the play equipment and the interior surfaces of the shelving units.3) The rugs located in the Toddler Room and Pre K Room were observed to be dirty and contain visible particles.Please deep clean the rugs.4) Large holes were observed on the couch cover located in the Before and After School Care Room (left side).Please repair or replace the couch cover.5) The wall vent at the daycare entrance was observed to be loose and not properly secured.Please reinstall the wall vent to ensure it is securely fastened.6) The filter of the air purifier located in the Preschool Room was observed to be covered in dust.Please replace the filter.

Remind staff that mouthed toys must be disinfected between uses, or at least once daily.

      • The crib mattress located in the Infant room was covered with a cloth mattress cover that was not impervious to moisture.Please use a cover that is easily cleanable and moisture resistant.
