      • A ceiling light cover was observed to be hanging and did not appear to be seated properly in its fixture. Ensure the identified light cover is repositioned and securely fitted to prevent it from falling and creating a potential safety hazard.

      • -Gaps were noted near window openings- weather stripping was not adequate.Pest control methods were discussed. Ensure that all window gaps are sealed, and weather stripping provides an effective seal.

      • Bleach/chlorine sanitizer was measured at 500ppm with bleach/chlorine test strips. Ensure bleach/chlorine sanitizer is maintained at 1000ppm during gastrointestinal outbreak. The operator prepared a 1000ppm bleach/chlorine sanitizer during the inspection.

      • Two chlorine bleach sanitizer spray bottles were found stored on a low table accessible to children.The operator removed the bleach bottles to a shelf in the storage room that is inaccessible to children.

      • -A small, damaged area was noted on the wall in the boys washroom, revealing underlying materials. -Some wall bases along the walls in the carpeted play area were detached.Please repair the walls to ensure surfaces are smooth and easily cleanable.

      • Garbage and recycling bins were found full and uncovered.Ensure all waste is properly contained, kept covered, and removed regularly.

      • -Several large black flies were found throughout the facility.-Gaps were noted in several entrance doors to the facility.-Gaps were noted near window openings- weather stripping was not adequate.-Garbage and recycling bins were found full and uncovered.Pest control methods were discussed. The operator indicated that wasps are an issue in the front area; however, no wasp activity was observed at the time of inspection.1. Please increase cleaning and sanitation to control vermin.2. Ensure all waste is properly contained, kept covered, and removed regularly.3. Install pest control devices in the facility as needed.5. Ensure that doors leading to the facility remain closed, and that any gaps in doors leading to the facility are properly sealed.6. Ensure that all window gaps are sealed, and weather stripping provides an effective seal.
