      • Tools are not being fully dried prior to placing in the high-level disinfectant. A drying step is needed before disinfection to prevent diluting the solution and weakening its effectiveness.
    • 20. Is reusable equipment disinfected properly?
      • Records indicate the CS20 solution is only being changed once a month, not every 14 days as required by the manufacturer. Ensure the date of pouring into the container for use is recorded accurately and the solution changed every 14 days.

      • Written procedures for skin cleansing were not completed, but a copy of the template was on site. Operator completed the procedure during the inspection. Lash service provider must have procedures specific for that service.Ensure written procedures are reviewed and followed by all staff. Ensure procedures are updated if there is a change of product for cleaning the skin.

      • 1. No testing of high-level disinfectant could be validated as no records of testing were being completed. Last records were dated for Aug. 2025. 2. operator could not verify solution was changed within the 14 day resuse time. Operator changed solution and completed records for high-level disinfectant during inspection. Ensure CS20 is being changed every 14 days, solution is tested each day prior to use and results are recorded in accordance with the Personal Service Regulation and Standards.

      • Update: Operator yet to obtain test strips. Records are now being completed when solution is in use. A container was obtained for the foot grater but was not large enough to fully submerge the tool. Test strips were expired and may no longer be accurate. Please obtain new test strips for the high-level disinfectant.Foot graters do not fit in the container used for soaking semi-critical items in the high-level disinfectant. Please obtain a container that fully submerges the entire foot grater.

      • Eyelash services have been added without notifying AHS of the additional service. Please complete a PSN to notify of the additional service.
    • 20. Is reusable equipment disinfected properly?
      • Alcohol was being used as a disinfectant on environmental surfaces. Only disinfectants with a DIN and disinfection instructions can be used. Please discontinue use of alcohol and switch to a low-level disinfectant for use on tables and chairs. Test strips were expired and may no longer be accurate. Please obtain new test strips for the high-level disinfectant.Foot graters do not fit in the container used for soaking semi-critical items in the high-level disinfectant. Please obtain a container that fully submerges the entire foot grater.Records were incomplete and not routinely taken of testing the high-level disinfectant.No original container was present for high-level disinfectant to verify if remaining solution was expired.       • Sharps are being stored in a glass jar with lid, but disposed out of the container into the garbage. Operator advised to acquire a proper sharps container for storage and dispose of them in the container or at a pharmacy exchange.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Chemicals were being refilled into an unlabeled container. Operator was instructed to label the contents during inspection.

      • No records are being taken or retained for the high-level disinfection. A copy of the high-level disinfection log will be provided with a copy of this report. Operator must complete the required fields each day that the disinfectant is used.
