    • 08. Is this facility properly constructed (Non-Critical)?
      • 1-Baseboards throughout the facility were coming off the walls and damaged. Repair/placement required.2-Floor covering was damaged in the playroom near the Tv and missing at some points in Playroom.3-Ceiling tiles were noted being water stained and dislocated. Replace the water damaged ceiling panels. Investigate the source of water damage and fix it prior to replacing the ceiling panels.3- There were gaps noted around several ceiling panels. Properly install the ceiling panels, Some tiles were noted being missing from the ceiling. Install the missing tiles.4-Paint was chipping off the walls. Repair required.5-Wall was noted being damaged near the 2-compartment sink in the play room.

Resurfacing, repair required to facilitate cleaning and sanitization of the surfacing.2- A stuffed cushion was noted with ripped cover making it difficult to clean and washed in the washer. Repair required.3-A small kid bench was noted with repaired surfacing. Operator planning to get rid of the bench. Remove the bench or resurface it.

      • A broken Tv was not being improperly stored on the bench in the facility. Operator reportedly planning to discard it.Remove the Tv from the bench.
