    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for skin cleansing, antiseptic use, post-service care, accidental skin cuts and punctures, and decontamination of the reprocessing and hand hygiene sinks were not established.These procedures were completed using the AHS written procedure template at the time of inspection.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • *** Alcohol based hand gel was available but was expired since 10/07/2023.*** Hand washing sink was not supplied with paper towels. Corrective actions:- Please acquire ABHR that is not expired.- Install a covered paper towel dispenser by the sink to allow for drying of hands, while protecting the paper towels from contamination.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Client contact information was collected and retained for all services, including microneedling.*** Sterility assurance documentation for microneedles were not collected and retained with the client's records.Corrective Action:- Ensure all sterility assurance documentation for microneedles used during each service are kept with the client's records. This includes expiry dates, lot/batch numbers, and manufacturer's information.
    • 20. Is reusable equipment disinfected properly?
      • * Reusable semi-critical equipment such as extraction loops were cleaned and disinfected with Barbicide, which is a low level disinfectant.Corrective actions:- Ensure semi-critical equipment is disinfected with a high level disinfectant.- High level disinfectant have reuse periods which require its efficacy to be tested each day the disinfectant is used, through the use of test strips. Acquire the test strips that are appropriate for the high level disinfectant used.- Test, verify, and document the results of the test strips each day the disinfectant is used.
    • 24. Are waste sharps disposed of properly?
      • *** Sharps container was available; however it was filled past the maximum fill line.Corrective action:- acquire a new sharps container and ensure its contents do not exceed the maximum fill line.- place the sharps container in an area that is easily accessible, limiting the number of steps required to safely dispose of waste sharps.Note: used microneedle tips were discarded after they were placed back in their original plastic/paper packaging. Going forward, microneedle tips do not need to be placed back in their original packaging disposal.
