      • Stargazers 1 - Bleach bottle at diapering station was not operational. Spray bottle was replaced and labelled right away with bleach solution working.Ensure bleach bottles are working prior to operation.

      • Explorer 2: The area carpet was ripped/worn.Replace or remove the carpet. Ensure all finishes are in good condition, easy to clean.

      • Toy washing procedure was discussed with staff on site. It was indicated that mouthed toys are completed in sink in room (diapering hand sink). After discussion with additional staff - toy washing procedure is to be done in Stellar (OSC) room using 3 bin/sink method.Several rooms were noted to have dust/debris in the bottom of the toy bins. Ensure all bins and toys are being washed when soiled and at least weekly.*Please review the toy washing procedure for all rooms, including mouthed toys to ensure that the risk of cross contamination is minimized. Provide written procedure for staff on toy washing procedure including schedule.
