- The weather stripping on the back exit door is in poor condition, allowing visible light to pass through a gap. Repair or replace the weather stripping to prevent pest entry into the facility. When properly installed, no light from outside should be visible inside the daycare.

- A cabinet containing various items, including chemicals, was not properly secured to prevent child access. The operator had used a rope to tie the door handles together during inspection. Install a proper locking mechanism to ensure the cabinet is safely secured.
- 06. Are proper protocols being followed in regards to client care?
- The diaper changing pad in the baby room has a few small rips in it, so the operator had it covered with a bed sheet. However, the facility only had one bedsheet, so children were being diapered on the same bedsheet with no disinfection in between. Upon request, the bed sheet was removed, and the staff were instructed to disinfect the diaper changing pad with 100 ppm bleach water between children. The ripped changing pad should be replaced ASAP. The operator states she has ordered a replacement that should be arriving soon.Oct 24, 2025- The ripped diaper change pad and bed sheet were still in use in the baby. The change pad was placed on top of a new diaper pad purchased by the operator.- A cloth fabric was observed underneath the diaper changing table in the washroom, which may interfere with proper cleaning and disinfection of the area.Action required: - Remove all fabric coverings from diaper changing stations located in both the baby room and the washroom. All diaper changing surfaces must be non-porous and easily cleanable to allow for effective disinfection.- Replace the damaged diaper changing pad in the baby room with a new, intact pad to ensure the surface remains safe and easy to clean

- The floor transition strip in the OSC room is damaged. Repair or replace it to maintain a safe and cleanable surface.
- 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
- The paper towel dispenser at the hand sink was found to be empty. Staff immediately replaced the paper towels to restore proper hand hygiene supplies.
- 10. Is a properly supplied first aid kit provided and readily available?
- Some items in the first aid kits were found to be expired. Inspect all contents and replace any expired items to ensure the kits remain fully stocked and compliant.

- The lighting level in the OSC room is inadequate due to burnt-out bulbs.Replace the non-functional bulbs in the OSC room.

- The operator stated that diapering occasionally occurs in the washroom, but there was no disinfectant present in the washroom. Upon request, a bottle was brought to the washroom and stored appropriately. Please ensure disinfectant is always available when diapering.

- The far wall in the washroom is damaged near the bottom with exposed building materials. Please repair this.

- During a conversation about cleaning and disinfecting surfaces in the facility, the operator stated staff are instructed to spray surfaces with disinfectant (bleach water at 100 ppm), then wash surfaces with a wet and soapy cloth. The operator was requested to reverse this process, as surfaces should first be cleaned with a wet cloth to remove any visible dirt/debris, then disinfected and allowed to air dry. The operator was compliant with this request.
