      • The product (Rejuran) unapproved by Health Canada, is being offered by the operator to clients as an injectable or via microneedling therapy.**Discontinue the use of this product as an injectable and during microneedling therapy. -The operator removed the products from the shelves during the inspection.

      • There were no written procedures for application of antiseptics, cleansers and decontamination of a dual-purpose sink.**The operator filled out the templates during the inspection. **Please update the templates whenever required.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation of pre-purchased sterile equipment is not being attached to client records.**Ensure to attach the sterility assurance documentation to client records which should at least indicate the name manufacturer, type of equipment, lot number, expiry dates and method of sterilization.
    • 20. Is reusable equipment disinfected properly?
      • -Test strips for the high level disinfects on site was expired.-Daily testing for the high-level disinfectants is not being carried and testing records are not being maintained.**Ensure to purchase valid test strips.**Ensure to test the disinfectant daily and maintain adequate records on site. The disinfectant must be used according to the manufacturers instructions.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • The reprocessing area was cluttered and unorganized with no observable pattern of workflow or reprocessing. **Have the reprocessing area decluttered and organized to ensure a unidirectional flow of work to allow for proper reprocessing and to prevent the cross contamination of reprocessed equipment.
