- *** Reusable equipment was not being disinfected with a high-level disinfectant.Corrective Actions:- Please acquire a high level disinfectant and appropriate test strips.- Follow the manufacturer's instructions for use, including reuse days, contact time, and rinsing procedures.- Test the efficacy of the disinfectant each day it is used, and document the results. Use the two copies of the AHS high level disinfectant log that were provided during the inspection.

- *** Written procedures for skin cleansing and antiseptic use were not established nor available.- PHI provided assistance and completed the required written procedures with the owner during the inspection. Ensure these procedures are placed in an area that is visible and accessible for workers.

- *** Previously used, single use disposable items such as lash curlers, pumice stones, and emery (nail) files were stored in UV cabinets.Corrective Actions:- All porous, single use, disposable items must be discarded after one use.

- *** Tweezers and scissors used for waxing were stored in the client service room in a visibly dirty condition. They were contaminated had wax, hair and other debris.Corrective Actions:- Please ensure these items are always taken to the reprocessing area to be cleaned and disinfected, before they are placed back into storage.- Ensure all previously used reusable equipment is cleaned and disinfected after each client service.
