- 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
- There was no soap in the handwashing dispenser located in room 2. **Soap was replaced by operator. **Handwashing sinks must always have running water, soap, and paper towels.
- 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
- 1. There were several buckets of bleach solution in all rooms that had concentrations between 0-50ppm when tested. 2. When asked, there were no test strips accessible to verify the concentrations of bleach solutions in bottles and buckets. **Bleach solution should be at 100ppm and can be made by diluting half a teaspoon of bleach in 1 liter of water. Test strips must always be accessible to ensure appropriate concentrations. 3. Several wiping cloths were observed on counters and submerged within water. When asked, employees advised they use the water to wipe tables after disinfecting with the bleach solution. **Wiping cloths must always be stored completely submerged within disinfectant solutions at appropriate concentrations. **After wiping a surface with bleach solution at 100ppm, do not rinse with water. A rinse with water is recommended only when using bleach solutions at 1000ppm.

- 1. There was a couch in room 1 with significant rips/tears. **Replace/repair/remove the couch to ensure that all surfaces remain smooth and cleanable.

- In room 5, the sink used for handwashing and also used as the washroom sink was also being used for toy washing.**There must be at least one easily accessible sink for washing toys. It cannot be the same sink used for handwashing, and it cannot be a washroom sink.

- The staff tap in room 1 was observed to be in disrepair. **Have the tap fixed by a technician to improve accessibility to handwashing facilities.

- In Room #1, there were bunny stickers which children were able to use which are not meant for children < 3-year-old (due to being a small choking hazard). Room #1 is a toddler room.Director removed the stickers from this roomREQUIREMENT: Follow all manufacturer's requirements on toys. Ensure all toys, arts & crafts items, and other child accessible objects are age appropriate.

- An electronic elephant was noted in Room #1. This toy cannot have it's electrical components removed and therefore cannot be routinely laundered.Director removed the toy from the room.REQUIREMENT: Ensure all electronic toys are able to be cleaned, disinfected, or laundered and hot air dried.

- In Room 6A (right side), there are several blue sleeping cots which were visibly stained. These cots were fabric and may need to be laundered.In Room 6B, there were vinyl tan sleeping cots which were not being used. The inspector asked the Director to switch out the cots to ensure that each child has clean bed and an easily cleanable bed.REQUIREMENT: Ensure all cots/beds are able to be easily cleaned and sanitized.
