    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Operator is now aware to keep sterility information with client information for up to 2 years and indicated they will complete this record keeping from now on - including for microneedling,
    • 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
      • One time use nail files (emery board) are re-used between clients. Emery board file is an uncleanable surface. Please discard / provide to client after each use.Emery boards are still re-used between clients. Ensure emery boards are not re-use between clients.

    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • Provided operator a copy of template for written procedures. Please fill out and return to the PHI.
    • 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
      • One time use nail files (emery board) are re-used between clients. Emery board file is an uncleanable surface. Please discard / provide to client after each use.
    • 20. Is reusable equipment disinfected properly?
      • An unapproved sterilizer was on site used for tweezers and nail equipment by nail/lash tech. Discussed that sterilizer does not have a way to measure time, temperature or appropriate pressure (eg. printing machine). Operator stated that they will switch to CS20.CS20 is used appropriately where required. Records of CS20 for re-use was no complete. Provided operator with high level disinfection logs, and provided training on using test strips (follow manufacturer instructions). Pre-empt wipes used for non-critical equipment.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • Sterilizer on site used primarily for tweezers by the lash tech. Sterilizer is unapproved and does not provide records of time, temperature and pressure. Operator will switch to CS20.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Mixed reconstituted botox in the fridge, and bacteriostatic sodium chloride solution on site for use. Manufacturer instruction was not being followed.Manufacturer instruction for use indicates the following Reconstitution and Handling Instructions1. Diluent: Use only 0.9% sterile, non-preserved Sodium Chloride Injection.2. Dilution: For a 100-unit vial, add 2.5 mL of saline to yield 4 units per 0.1 mL. For a 50-unit vial, add 1.25 mL.3. Mixing: Gently swirl the vial; do not shake vigorously, as this can denature the protein.4. Storage: Store reconstituted vials in a refrigerator at 2-8C.5. Usage Window: Use within 24 hours of reconstitution.a)Botox must be reconstituted with saline with no preservatives (no bacteriostatic sodium chloride solution is permitted) b) Solution must be used within 24 hours of reconstitution
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Reprocessing area has microwave and also appears to be use for staff room. Reprocessing area must only be used for cleaning instruments. Microwave to be removed from reprocessing area.
