- Bleach sanitizer was measured to be 0ppm chlorine. A new solution was prepared immediately upon request measuring 100ppm chlorine.

- 1. Bowls were being used as scoops and stored inside bulk food items. Use scoops with a handle and store in a way that protects food from contamination. 2. Onions were being stored in a box on the floor of the storage area. Store onions and other food items off the floor.

- The handwashing sink cabinet was repaired with duct tape. Properly repair handwashing cabinet so that it is smooth, non-porous and easy to clean.
