      • Unreported services, microneedling, injection services, lash tinting and radio frequency skin treatments were conducted without written procedures in place.Jan 21, 2026The operator has the procedures printed out on site. The procedure will be emailed to the inspector for a review.

      • The disinfection protocol for the reusable equipment was not set up. The soiled area and the sanitized area were mixed together.Jan 21, 2026The disinfection area and the soiled area are clearly separated now. In the lash tinting equipment rack, one soiled equipment was found stored inside the clean equipment bottle. CS20 record was found showing use beyond 14 days. CS 20 after pour can be only used no more than 14 days according to the manufacturer's directions if concentration is sufficient.

      • Botox service and other injection service were provided. However, storage of the products cannot be verified. Botox products cannot be found on site. Unapproved exosome products, Curestem C20, bottles were found on site. This contains human cord blood cell and cannot be used. Cease the injection services until approved.Jan 21, 2026Met with the injection nurse, Parmeet Gill, on site. There are two brands of botox available. One has DIN and the other does not. Only Health Canada approved products can be injected. Botox products were reconstituted with preservative containing saline. Follow the manufacturer's instructions, only preservative free sterile saline can be used for the reconstitution.
