- Written procedures templates were not prepared at the time of the inspection.

- Operator was unfamiliar with Personal Services Standards. Education provided during and after the inspection.

- Hand sanitizer was expired in 2020. Please purchase new hand sanitizer that is at least 60% alcohol content and is labelled with a DIN or NPN.

- Clients were not being provided with written post-care instructions. Verbal instructions were given.

- Operator was not retaining any client or needle sterility information. Education provided on the information that must be retained in records.

- 99% alcohol was being used to disinfect tattoo machine and service bed. Alcohol is not a permitted disinfectant. Education provided on low-level disinfectants.
- 24. Are waste sharps disposed of properly?
- Used needles were being disposed of in a regular trash can with no lid. Education provided on requirements for a sharps container.
- 26. Are cosmetic products appropriate for their intended use and being handled properly?
- Tattoo ink was being diluted with tap water that had previously been boiled. Education provided about sterile water requirements for dilution of ink.
- 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
- Service area is carpeted. Education provided on appropriate surfacing for service area. Service area must be non-porous, easily cleanable and smooth.
