- - Both bacteriostatic and preservative free saline were observed in the facility.** The facility is using bacteriostatic saline for reconstituting neuromodulators such as botox. One staff mentioned that the bacteriostatic saline makes it less painful for the client.- The neuromodulators were properly stored in a refrigerator and daily temperature records of the refrigerator unit were observed.- The operator stated that reconstituted neuromodulators like Botox are used within 24 hours.- The facility has nurse practitioner and medical directors that make prescriptions for their clients. All clients' intake forms and treatment information are captured and stored on TouchMD.ACTION REQUIRED1) The manufacturer's instruction for reconstituting the neuromodulators must be followed. - Provide the manufacturer's instructions for use for the neuromodulators that state that they can be reconstituted using bacteriostatic saline. In the absence of such instructions, the neuromodulators like botox shall be reconstituted using preservative free saline.
