    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No - facility specific written procedures were not established for skin cleansing, antiseptic use, and post-service care for invasive procedures.An AHS written procedure template package was left with the operator, along with an explanation of each required section.Please ensure the required written procedures are established for this facility.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • No - sterility assurance documentation for all critical equipment used during each invasive procedure was not being collected and retained.Please ensure sterility assurance documentation (lot#s, expiry dates, etc.) is collected and retained with the client's records for every invasive service.
    • 20. Is reusable equipment disinfected properly?
      • No - the following expired disinfectants were observed:- Accel CS20;- Caviwipes x2;- Cawipes 2.0; and- Caviwipes 1.Operator was claiming to use the Accel CS20 as a surface disinfectant.Please ensure an appropriate intermediate level disinfectant is acquired for use on non-critical equipment and surfaces.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • No - critical, single use equipment, including but not limited to the following were observed to be expired:- Blood collection needles;- Insulin Syringes; and- Injection plugs. Ensure all expired critical equipment is safely removed from the facility. Remove any equipment that is no longer used for the services you are currently providing to the public.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • No - numerous cosmetics were expired and had inadequate labeling. This includes but not limited to the following:- Container of topical xylocaine expired since 08/2011;- Cahnsai Sheep Placenta Oligopeptides Freeze-Dried Powder which contained no English instructions or list of ingredients; and- Bacteriostatic Saline. Observed numerous partially used vials and ampoules of cosmetics stored in the client service area including but not limited to the following:- Stayve Whitening Step Cell Culture; and- Be Balance ALA-13. Ensure all cosmetics used in the facility are not expired, and clearly labeled with information about the product identity and manufacturer, including a complete list of ingredients.
