- Pest monitoring was not being completed monthly. Ensure pest monitoring is completed monthly and records are kept and easily accessible.

- A floor tile located in front of the grease trap was missing. Replace the missing floor tile. Ensure the surface is smooth, durable, and impervious to moisture to facilitate ease of cleaning.

- Dust and dirt build up was observed on the walk-in cooler fan cover. Some white residual build up was observed on the inside of the ice machine. Clean the above mentioned areas.

Grey/and black residue build up was observed on the flooring tiles along walls and corners of the facility. Deep clean the floor throughout the facility.

- The low temperature dishwasher had 0ppm of chlorine after priming and several cycles. Sanitizer line was not flowing. Operator indicated it will be corrected by a technician before the end of day.Using the 2-compartment sink at the back of the kitchen,then sanitizing in 100ppm of chlorine souluton for at least 2 minutes was discussed with the operator. The operator was also given the option of using single use utensils for service until dishwasher can be repaired. Ensure dishwasher is repaired and tested at 100ppm of chlorine.
