    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Pest monitoring was not being completed monthly. Ensure pest monitoring is completed monthly and records are kept and easily accessible.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • A floor tile located in front of the grease trap was missing. Replace the missing floor tile. Ensure the surface is smooth, durable, and impervious to moisture to facilitate ease of cleaning.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Dust and dirt build up was observed on the walk-in cooler fan cover. Some white residual build up was observed on the inside of the ice machine. Clean the above mentioned areas.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • A deep clean appeared to have been completed for the walls and ceilings. However, the walls and ceiling tiles appeared to be stained/discolored. Grey/and black residue build up was observed on the flooring tiles along walls and corners of the facility. Deep clean the floor throughout the facility.

      • Walls, ceilings and floors are starting to stain/build residual due to age.Within the 6 months schedule a deep clean of all surfaces (Walls, Floors and Ceilings), move equipment, re-organize and repair damaged surfaces, remove equipment no longer in use.

      • No (outstanding) - there was inconsistent record keeping in the staff use of daily logs/checklists of food safety temperatures for all cold-holding, freezer units, and hot-holding units, as applicable.Staff are to complete daily food safety temperature checks (note: AHS copy of 'record-keeping template' was provided to operator by email).

      • No (outstanding) - there was inconsistent tracking of monthly pest controls records. The operator and staff are to keep consistent records of all monthly pest control records (whether by staff use of the 'AHS monthly pest control checklist' or completion of pest control monitoring by a licensed pest control company). These records are to be kept on-site, available for review, for a period of one year.Copy of 'AHS monthly pest control checklist' was sent to operator by email.

      • What appears to be dead cockroaches were observed in the handwashing sink. It kept coming back up the drain. Cockroaches were not observed anywhere else.Ensure you keep an eye on it. Contact a pest control company as needed.

      • Lights in the kitchen prep area were missing covers.Ensure covers are replaced.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The indicated areas are unclean;1. The walls and ceilings of the walk-in cooler and walk-in freezer were unclean.2. The inside of the ice machine. Dumping out the ice, then cleaning and sanitizing the ice machine with 100ppm of chlorine solution was discussed.Ensure the indicated areas are maintained in a sanitary manner at all times.

    • 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
      • The low temperature dishwasher had 0ppm of chlorine after priming and several cycles. Sanitizer line was not flowing. Operator indicated it will be corrected by a technician before the end of day.Using the 2-compartment sink at the back of the kitchen,then sanitizing in 100ppm of chlorine souluton for at least 2 minutes was discussed with the operator. The operator was also given the option of using single use utensils for service until dishwasher can be repaired. Ensure dishwasher is repaired and tested at 100ppm of chlorine.
