      • The facility does not have written procedures established to describe safe and sanitary practices such as:- preparation of skin- post-service care- client care in events of skin cut or puncture- Dual sink procedures
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • A lack of proper documentation of lot numbers of products was observed not being included in clients' waivers. This is an effective way at tracking the products used on that specific client. Please ensure that all waivers contain lot numbers.

      • Green soap is expired as of 2025. Please replace the green soap and send a photo to verify that this has been completed.

      • No antiseptic products were available. The owner made arrangements to have an individual purchase 90% rubbing alcohol during the inspection. Valid, rubbing alcohol was brought back to the facility with an expiry date of 2030.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Green soap is expired as of 2025. Please replace the green soap and send a photo to verify that this has been completed.

      • Some of the tattoo inks, were expired. The owner discarded them and made note to look through inventory more frequently.

      • One of the five beds had a small tear on the leather bed. The operator has plans for rewrapping the bed and will not be using the bed until it is repaired.

      • One of the five beds had a small tear on the leather bed. The operator has plans for rewrapping the bed and will not be using the bed until it is repaired.

      • The handwashing sink was not equipped with a sanitary option for drying hands such as paper towels. After the conversation with the operator, the paper towel was placed in the holder. Please ensure handwashing sinks are equipped with soap, paper towels, and hot and cold running water for effective handwashing.
