      • 1. Chemicals are stored under one of the children's bathrooms, and the childproof lock was not secured.Consider replacing the childproof lock with a keyed lock for improved safety, as children aged 5–6 may bypass childproof mechanisms. Alternatively, store chemicals in an area out of reach of children.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. Blue room: There was an aerial silk station that has not been previously reviewed or approved by AHS. The pillow surfacing also does not look like it provides a large enough area to protect from falls.Please remove the aerial silk station since it has not been approved for use.

      • 1. Staff were unable to explain the cleaning and sanitization process for the natural wooden blocks.If the wooden blocks cannot be properly cleaned and sanitized, they must be discarded.

      • 1. The cleaning schedules for the red room and blue room were incomplete and not available for review during the inspection.Please ensure staff are completing the checklist on a daily basis.

      • Some milk bottles were not labeled with a child's name in a yellow room. - All milk bottles must be adequately labeled with a child's name.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Some storage bins for children personal belongings were dirty in the yellow room. - Clean regularly.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • A portable sink with a change table was installed in the green room without prior approval. Portable sinks are not allowed since the National Plumbing Code requires all fixtures be directly connected to the sanitary drainage system.All handsinks must have potable water. - Do not use the portable handsink for handwashing/bib washing/toy washing. Use the main change table by the washrooms or in the yellow room for diapering.

      • There was clutter around toilets in the yellow room.- Remove clutter to allow for adequate cleaning.

      • 1. There was only cold water (11C) at a handsink in the red washroom. Blue washroom hot water measured only 23.2 C. - Ensure that both washrooms have adequate supply of hot water for handwashing. * Hot water tank will be upgraded*

      • 1. A window in the infant room was not shatterproof or was not blocked. - Block the windows
    • 06. Are proper protocols being followed in regards to client care?
      • 1. No disinfectant was available at a change table close to washrooms. - Provide 2. No liner was used under a child during diapering. - Corrected. Train staff on proper diapering procedure.

      • 1. Surface on a door in red rooms was damaged. - Repair so the door is moisture resistant and easy to clean. 2. Bottom of mirrors at washroom sinks was water damaged. - Repair so the surface is moisture resistant and easy to clean. 3. Celling vents were dusty. - Clean regularly

      • 1. Staff was unsure how to properly clean and disinfect toys. Staff was unsure how to mix bleach solution for toys immersion. Some staff only disinfect toys without washing them first and some staff was mixing soap and bleach together. - Train staff on proper toy washing. Toys must be clean first with soapy water, rinsed and then disinfectant by approved disinfectant. 2. Balls in a ball pit are being only sprayed with a disinfectant. No adequate regular cleaning of balls is performed. - Provide a written procedure for cleaning and disinfection of balls in a ball pit.

      • 1. There was only cold water at handsinks in washrooms. - Restore hot water at the handsinks.

      • 1. Hats and gloves were stored on top of each other in blue room. - Keep personal belonging adequately separated to prevent cross-contamination.
