      • Pest control records were not observed during the inspection. Empty AHS pest control sheets were noted during the inspection.The operator indicated that they do not maintain pest control records. Please be advised to maintain pest control records on monthly basis.
    • 17. Are general nuisance conditions, other than those noted in other violations, being prevented at this food facility (Critical)?
      • The facility was performing toy washing in the commercial dishwasher at the same time as of food prep. Soiled dishes were noted throughout the counter in the kitchen and arts and crafts material noted inside the dishwasher.Please note that the toy washing must occur once the food activities are completed for the day.

      • 1. Frozen containers of spaghetti were noted in the freezer. When inquired, the operator indicated that these were leftovers from their Christmas party.Please discard all these containers. Please be advised all food items served to clients are acquired from approved sources. Please only serve cold food items to clients as per facility's approved menu.2. It was noted that the facility changed their food menu without approval. The facility is approved for cold food items only. New menu included soups, pasta and lasagna. The facility does not have a stove to prepare these menu items. When inquired about the change of menu, the operator indicated that they thought they were allowed to change menu without approval. Please adhere to the approved food menu. Any changes to the food menu or equipment must be approved by AHS EPH.3.
