   • 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
     • Single-use wooden wax applicators were observed being reused.- Ensure all wooden wax applicators are single-use only and are discarded after each application.
   • 20. Is reusable equipment disinfected properly?
     • 1) A new solution of barbicide was not prepared according to the manufacturer (too dilute), and the solution did not fully submerge the tools inside the container. REQUIREMENT: Barbicide must be prepared in accordance with the manufacturer's instructions. Ensure enough solution is prepared to fully submerge the non-critical reusable tools.2) The Preempt CS20 and RTU disinfectants were both expired. The operator was not offering comodone extractions at the time of the inspection.REQUIREMENT: Disinfectants must be checked to ensure they're not expired. Until new disinfectant is purchased, services that utilize reusable semi-critical equipment such as comodone extraction cannot be offered.
   • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
     • The facility was dirty, dust accumulation and hair were observed on all tools, equipment's, and shelving unit.- Thoroughly clean and disinfect the facility.
   • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
     • The mat under the second barbing chair was badly thorn and exposing foam material making it uncleanable.- Repair the mat or replace. The surface must be made of material that is smooth and easily cleanable.
