    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • A personal service notification (PSN) has not been submitted- PHI will provide link to submit a PSN
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • There are no written procedures prepared- Ensure written procedures are completed for the following areas: Skin cleansing before nail and waxing services, accidental skin cuts or punctures.- PHI will provide templates
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • There is no skin cleansing step before waxing services- Ensure skin is cleansed before waxing services
    • 20. Is reusable equipment disinfected properly?
      • Isopropyl alcohol is being used to disinfect metal nail file pieces- Ensure a suitable disinfectant with a DIN or MDL is being used to disinfect tools- Reusable metal nail files must also be properly washed before disinfection not only debris brushed away.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • The sink used for cleaning and reprocessing semi-critical tools is also used for dishwashing and food prep- Ensure the reprocessing sink and area is separate from contamination from other activities and not accessible to clients
