    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • 1. Spoolie observed at the workstation appeared to be reused. Spoolies are single-use items and must be discarded after each client to prevent cross-contamination between clients. Spoolie found at the wax station was discarded during the inspection.Any single-use items, including spoolies, must be discarded properly after each client and not reused.2. Roll-on wax cartridges were observed at the wax station. The operator stated the cartridges were no longer being used and were removed from the service area.Ensure items not being used are removed from the service area to prevent any inadvertent use.
    • 18. Is reusable equipment cleaned properly (Non-Critical)?
      • The cleaning tools were not maintained in good repair. The bristles were frayed. Cleaning tools must be maintained in good condition to ensure semi-critical tools are adequately cleaned prior to disinfection. Obtain new cleaning tools.
    • 20. Is reusable equipment disinfected properly?
      • 1. Semi-critical tools such as the nail clippers and cuticle clippers were not fully submerged in the high-level disinfectant solution. The operators were using Preempt CS20 as their high-level disinfectant solution. The manufacturer's instructions must be followed accordingly to achieve proper disinfection. Ensure all parts of the semi-critical tools are completely submerged in the high-level disinfectant for 20 minutes (according to the manufacturer's instructions). Refrain from over filling the container with the high-level disinfectant.2. The interior of several manicure curing lamps were soiled with debris.Equipment must be thoroughly cleaned and disinfected in between clients. Ensure the interior of the curing lamps are regularly cleaned and disinfected.3. The operators are reusing the high-level disinfectant for more than a day. No high-level disinfection logs were available to review at the time of the inspection. Disinfection logs must be completed everyday when reusing the high-level disinfectant solution. Complete a high-level disinfectant log. An AHS high-level disinfectant log template was provided to the operator via email along with this inspection report.
    • 20. Is reusable equipment disinfected properly?
      • 1. The high-level disinfectant (Preempt CS20) was expired in April 2026. The high-level disinfectant must not be expired to ensure adequate disinfection of semi-critical tools. Ensure the expiration date of the high-level disinfectant is regularly monitored. The operator purchased a new high-level disinfectant (Preempt CS20) at the time of the inspection.2. There was hair and debris lodged inside of several jet impellers for the pedicure basin. The jet impeller must be taken apart and thoroughly cleaned to remove contamination then disinfected. Ensure the jet impeller is thoroughly cleaned visually inspected for cleanliness prior to disinfection. The operator cleaned and disinfected the jet impellers at the time of the inspection.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Visible hair was observed within the wax pot. Contaminated wax poses a risk of biological contamination & cross-contamination between clients. The contaminated wax pot was discarded during inspection.Maintain wax pots in a clean & sanitary condition by avoiding topping up & ensuring wax pots remain covered when not in use.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. Visible dirt & debris were observed within the storage cabinet in waxing room. The cabinet used to store sealed wax pots was not being maintained in a clean and sanitary condition.2. Pre-cut waxing strips were stored in a container with visible dirt and hair present within the container.Clean and maintain storage cabinets, and container used to store waxing supplies in a clean and sanitary condition to prevent contamination of supplies.3. One wax pot stored within the cabinet was open, with wax residue adhered to the surface below, and the wax inside the pot was visible dirty.Remove the adhered waxy residue from the surface and discard the contaminated wax pot.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Several cosmetic products were decanted into another container and did not have a label. Cosmetic products must be properly labeled for staff awareness and safe usage of the product. The operator labelled the containers during the inspection.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Several of the manicure workstations had an accumulation of dust, debris, and nail clippings in the drawers. Workstations must be maintained in a clean and sanitary manner.Thoroughly clean and disinfect the manicure stations.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Brow tinting materials were observed within the waxing room cabinet. The operator stated brow tinting services were not being offered and the materials were strictly intended for personal use only. The brows tinting materials was removed from service area during the inspection.Ensure personal items and materials not intended for client services are stored outside the service area to prevent inadvertent use.

    • 17. Is reusable equipment cleaned properly (Critical)?
      • Several reprocessed nail clippers with visible residue and skin shavings were found stored in the cleaned tools cabinet.Ensure all tools are thoroughly cleaned and visually inspected after reprocessing. Use a brush to effectively remove debris prior to disinfection.
    • 20. Is reusable equipment disinfected properly?
      • 1. Staff were using 70% alcohol to disinfect work surfaces and tools.Cease the use of alcohol as it is not an approved disinfectant. Obtain a low-level disinfectant for non-critical surfaces and a high-level disinfectant for semi-critical tools.2. The Preempt RTU stored under the reprocessing sink was expired as of 2019.Discard this bottle. Ensure disinfectants are only used within expiration.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • There was no hand soap available at the hand sink in the washroom. Soap was temporarily brought in from the wax room.Provide a dedicated hand soap dispenser for this hand sink to ensure proper hand hygiene practices.
