      • The operator mentioned to have been conducting periodic pest control checks, but no pest control records were maintained at the time of inspection.- Please maintain pest control records on-site at all times.- Operator will be emailed AHS monthly pest control checklist.
    • 18. Are general nuisance conditions, other than those noted in other violations, being prevented at this food facility (Non-Critical)?
      • **Reoccurring Violation**The walk-in freezer door is completely broken. Staff have to physically lift the door to fully detach it, have someone else hold the detached door or set aside, then lift it back into place and lock it. - Please fix the door.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • 1. Worktable located next to the band saw was in disrepair as the corner was taped together.- Please replace or properly repair unit so surfaces are smooth, nonporous and easy to clean.2. Floor tile at the side delivery door was cracked, creating gap underneath the door.- Please fix.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The staff bathroom was cluttered with many miscellaneous items. - Please remove/declutter/re-organize.
