    • R4.1 - Are person in care records current, complete and kept confidential?
      • Observation(s): In review of records for 2026, monthly weights were not consistently being recorded. Lack of monthly weight documentation could affect the health of persons in care as this documentation may impact the ability to monitor changes in health status. Submit by May 29, 2026, a written plan outlining how you will ensure weights are being recorded monthly. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.
      • R4.1U - Weigh each person in care monthly and record their weight in their nutritional plan (Does not apply to Hospice); 83( 4 )(a)(c)
    • R4.6 - Are facility records current and complete?
      • Observation(s): There is not a process in place to monitor food and nutrition services provided by the Licensee. Ensuring there is a process in place to monitor the food and nutrition services provided by the Licensee works towards promoting the health of persons in care. Submit by May 29, 2026, a written plan outlining how you will monitor the food and nutrition services provided to persons in care. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.

    • R6.2 - Have all persons admitted to the facility complied with the Province’s immunization and tuberculosis control programs?
      • Observation(s): The system to ensure that all persons admitted have a TB screen on record and up to date immunization status on file appeared to be ineffective. In a random review of person in care records, licensing officer noted a person in care did not have TB screen on record, and a few persons in care did not have an immunization status on file.
      • R6.2A - Ensure that all persons admitted comply with the Province’s immunization and tuberculosis control programs; 49 ( 1 )
      • R6.2B - Keep clear and up to date records of the immunization status of each person in care; Director of Licensing Standards of Practice: Immunization of Adult Persons in Residential Care
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): In a review of records, licensing officer noted a person in care who frequently leaves the facility did not have a thorough elopement care plan in place. The care plan referred to the fact that the person in care may leave the facility, however the plan did not speak to how to locate the person in care, and when and how this should occur. In a review of records, licensing officer noted a behavioral care plan which had reference to information which was no longer applicable to the person in care.
      • R10.3G - Ensure the care plan for persons at risk of leaving the facility includes a plan to prevent the person in care from leaving and if the person leaves without notification, a plan to locate the person in care; 81( 3 )(f)(i)(ii)
      • R10.3K - Review and, if necessary, modify each care plan if there is a substantial change in the circumstances of the person in care, or at least once a year, to ensure it continues to meet the needs, preferences, and is compatible with the abilities of the person in care; 81( 4 )(b)(i)(ii)
