Facility is cluttered and disorganized in the staff areas (back storage room & behind cashier counter).2.       • Operator was unable to provide proof of purchase for the following items:-Ensure-Boost-PediasureCDI: Above items (approximately 100-140 bottles) were discarded in the building dumpster at the time of inspection.
    • 03. Is all food in this facility fit for human consumption and not unwholesome, spoiled, or adulterated?
      • High-risk, refrigerated foods were well past the best before date, namely:-Bacon, 2 packages BBD: April 28, 2025-Sausage, 5 packages BBD: May 16, 2025-French onion dip, 2 packages BBD: May 9 & June 13, 2025CDI: discarded the above noted items at the time of inspection.

      • A 4L bottle of Ortho Home Defense Max Insect Control was stored in the staff lounging area.ACTION REQUIRED: remove the insecticide from the facility.       • A pest control company has been hired to conduct treatment for cockroaches and implement a monitoring program for the facility. One live nymph was noted inside a bottle on a shelf above the sink at the back of the facility and was discarded. No further activity was noted on the glue boards currently present in the facility. **Still outstanding and first noted March 15, 2024: The weather stripping at the bottom of the backdoor is in disrepair- a portion of it has broken off. Repair/replacement is required to help prevent entry of pests.

      • **Openings/edges along the tiles and strips of metal that are installed on the wall above the sink in the back area must be sealed along their edges to prevent harborage from pests. **Baseboards that are either missing and/or falling of the wall under the sink area and next to the white refrigerator at the back of the facility must be repaired/replaced so that they are smooth/flush against the wall to prevent harborage of pests. A hole/gap was noted between the floor and the bottom of the kitchen exterior door.Fix or replace the weather stripping on the back exterior door to prevent pest (mice & insects) entry into the facility.

These items are to be removed as instructed previously. -Although not observed during today's inspection, the owner advised that he has a cat on-site at times "to catch mice". Owner was again reminded that live animals are not permitted to be in the store.-one live cockroach was observed crawling beneath the stove in the back kitchen area.Noted Feb. 4, 2025: 1. An unapproved animal was living onsite: --A live orange cat was observed walking freely within the store --A litter box was observed in the back kitchen area (southeast corner of the store) --A dish of food for the cat was observed next to the staff/office area (southwest corner of the store) ACTION REQUIRED: remove the cat & cat supplies.2. Mouse droppings were observed on the ground behind the retail counter in the front of the store (across from the cappuccino area). ACTION REQUIRED: clean up the mouse droppings. Consult with your pest control provider & follow recommendations. 3. A cockroach was observed on a sticky trap (on a shelf above the sink) in the back kitchen area.       • The facility was cluttered and disorganized behind the retail counter and in the storage areas making it difficult to clean & monitor for pest activity, for example: -Various supplies were stuffed into the cupboards under the cappuccino machines-Several boxes of stock were stored on the ground across from the cappuccino area -Garbage, bags of bottles, & boxes were stored in the back entry way by the east exterior door blockading the exit-General debris, garbage, and miscellaneous items were left on counters and in the sinks of the back kitchen areaACTION REQUIRED: conduct a deep clean & organization of the facility. Remove any equipment or supplies that are not required for the operation of the facility. Ensure stock is kept off the ground.

      • Bottles of Raid and Orthoinsecticide were found in the back room of the facility. A white powder was observed along the baseboards at the bottom of the walls in the back area of the facility. Note that chemicals for pest control are only permitted to be applied in food establishments by licensed professionals. Remove all pesticides/insecticides from the facility and ensure any chemical residue is cleaned-up.
