    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Sanitizer bottle in snack room was not labeled.2. Sanitizer bottle in preschool room was incorrectly labeled as "just water" when the contents were actually a surface disinfectant. Incorrect/lack of proper labeling poses a risk to clients/staff, as there is no way to identify the contents within these bottles. Please ensure that chemicals are kept in containers which are clearly labeled to identify the contents.
    • 03. Are clients at this facility being protected from hazards (Critical)?
      • Bleach sanitizer was tested and observed at a concentration over 1000 ppm. Test strip was bleached out after testing spray bottle of sanitizer in snack room. Inspector instructed director and other staff member at the time of inspection on how to make a proper solution at the correct concentration. Staff were able to make solution properly. Note that: 100 ppm is the required concentration for normal operation and 1000 ppm is required for outbreak. Please ensure use of test strips to verify concentration and that bleach solution is made daily.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • Alcohol wipes in both first aid kits (one located in snack room and one located in staff front reception area) were observed to have expired in 2010 and 2011. Please ensure both kits are stocked with new supplies to ensure that supplies are readily available for emergency response and treatment.
