      • A staff member’s personal cell phone was observed on the prep table where dough was being prepared. PHI advised the operator to discontinue this practice and ensure that no personal items are stored or placed in food handling areas to prevent potential contamination.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • No sanitizer test strips were available in the facility to verify sanitizer concentration.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • No hand soap was available at the staff washroom and the front handwashing sink, as the soap dispenser was not functioning. Staff confirmed that they had been using the handwashing sink in the food handling area after using the washroom. PHI instructed that this practice is strictly prohibited. The operator was advised to provide a temporary handwashing setup until the dispenser is repaired by maintenance.

      • Bulb holder inside the stand alone vertical freezer is not in good repair. Please ensure the dangling bulb holder is properly placed to ensure safe and sanitary operation of the facility.
