      • Current: Several worn sleeping mats remain in the childcare room and are still being used. The operator reported ordering approximately six cots and stated that replacing the remaining mats will take time. Previous: Several sleeping mats showed signs of wear and tear, with tape used to repair rips and missing sections of the outer layer. Damaged sleeping mats with tears and missing outer layers must be replaced, as tape does not provide a durable, cleanable, smooth, and tight seal necessary for adequate cleaning. Previous: Several sleeping mats observed throughout the program rooms are in disrepair. PHI observed tears and exposure inner material on several sleeping mats. Please ensure that sleeping mats exhibiting signs of disrepair are promptly replaced to ensure the presence of adequate cleaning surfaces and to promote clean and sanitary conditions for sleeping.

      • Current: Multiple light fixtures remain without covers or the use of shatterproof bulbs. Previous: Light covers are missing from several light fixtures, increasing the risk of a physical hazard (e.g., shattered bulb). To minimize the risk of physical hazards, please ensure all light fixtures have adequate light covers or shatter proof bulbs.

      • Current: The storage room near the Pre-kindergarten room remains unorganized and unclean. To mitigate pest harborage and maintain a sanitary environment, please ensure this room is thoroughly reorganized and cleaned.Previous: The social care facility's designated storage areas are significantly filled and disorganized, hindering adequate cleaning and creating potential pest harborage. The identified areas are a storage room near the pre-kindergarten room and a storage area near the laundry room.

      • - Play mats were observed to be visibly soiled during inspection.Action: Please ensure play mats are cleaned and maintained. Please ensure cleaning is included in cleaning checklist and is cleaned weekly or when soiled.-Carpets in the backroom (4+), need cleaning. Action: Please ensure it is deep cleaned and added to the cleaning checklist for monthly maintenance.- The 2-year-old room observed to be visibly unclean during inspection. Number of rags in the room not in immersed in sanitizer bucket. Action: Please ensure the room is thoroughly cleaned and sanitized. Please ensure cloths are immersed in sanitizer bucket, and/or paper towels are used in place with sanitizer spray(labelled).

Staff were alerted and the garbage was removed and discarded. Please ensure garbage and recyclables are discarded from the facility in a timely manner to minimize the risk of unsanitary condition increasing the risk of contamination to surrounding areas and illness.

      • In the bathroom of the toddler room, there is unfinished wall plaster near the sink - the affected area is approximately 2 feet long and 1 foot wide.Discussed with operator that unfinished plaster is porous and cannot be cleaned. She is agreeable to sealed the plaster to render the surface non-absorbent and easy to clean.

      • 1. Lighting fixtures in Pre-school and Toddler rooms (wall sconces) do not have covers. The glass bulbs are hanging exposed. Should a bulb explode, or an item break he bulb (such as a child throwing a toy up into the air), the glass bulb could shatter onto children and the play area.Please use appropriate covers for the light fixtures that protect the bulbs from damage and that would catch the glass should the bulbs break. Alternatively, another source of overhead lighting with protective covers may be used.2. The fluorescent light in the washroom (toddler room) does not have a cover.Please use a cover for the fixture. Alternatively, fluorescent bulbs in a plastic tube and shatterproof bulbs can also be used.June 12, 2023 update: Shatterproof light bulbs will be installed shortly.
