     • **Reusable manicure, pedicure, and waxing tools had buildup and rust on them, indicating that tools are not being properly cleaned, inspected, or maintained prior to disinfection and storage.ACTION REQUIRED:1) Ensure all reusable tools are thoroughly scrubbed with soap and water, rinsed, and inspected for cleanliness and physical condition (e.g., rust or damage) prior to high-level disinfection and storage.

     • - CS20 solution logs were not maintained.- The CS20 solution currently in use had been in the soaking tray since August 2026 (exceeding its maximum 14-day reuse manufacturer specifications).- The operator was unfamiliar with the required contact time (20 minutes for high-level disinfection) for CS20.- Although an unexpired bottle of CS20 solution was available on-site, the test strips used to verify the minimum effective concentration (MEC) were expired.ACTION REQUIRED:1) Establish a written, step-by-step cleaning and disinfection procedure (in accordance with the Personal Services Standards attached to this report) to ensure consistent staff compliance.2) Ensure that the manufacturer’s instructions for CS20 are strictly followed regarding reuse lifespan and required contact time.3) Maintain and document daily CS20 disinfection logs as required by the health standards.4) Obtain valid, unexpired CS20 test strips to test the minimum effective concentration of the solution.5) Implement a reliable timing method (such as a dedicated timer) to ensure staff meet the required 20-minute contact time for CS20 disinfection.

     • **Excessive accumulation of nail dust was observed inside all drawers across all manicure stations. Nail dust accumulation was also observed on client chairs and client contact surfaces, indicating inadequate routine cleaning and sanitation.ACTION REQUIRED:1) Immediately perform a thorough deep cleaning and disinfection of all manicure stations, including the interiors/exteriors of drawers, equipment, and stationary items at each station.2) Establish and implement a written, detailed cleaning program for routine sanitation.

     • **Proper cleaning protocols for manicure tools were not followed prior to immersion in CS20. Specifically, tools were rinsed under tap water without the required soap scrubbing to remove organic matter. Additionally, tools were not dried before disinfection, posing a risk of diluting the CS20 solution and compromising its efficacy.These discrepancies were reviewed with the manager, who has been instructed to ensure all personnel are fully trained and compliant with the complete reprocessing workflow.
   • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
     • - 70% alcohol was being used on workstations and contact surfaces; however, the container lacked a Drug Identification Number (DIN).- Staff failed to clean and sanitize a workstation between manicure clients.The manager was instructed to ensure all contact surfaces are properly sanitized between every service. While Mr. Clean Disinfectant is available on-site, it requires a 10-minute contact time to be effective. The manager must ensure surfaces remain visibly wet for the full duration. If high client volume makes this 10-minute window impractical, the facility should switch to a disinfectant with a shorter required contact time.
