      • No written procedures for skin cleansing prior to services, antiseptic use, and accidental cuts were available.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation for sterile equipment was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms. Discussed.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Sharp re-usable tools used for extractions during facials were observed. Ensure that all sharp tools used for extractions are discarded after one use. They were discarded during the inspection.
    • 20. Is reusable equipment disinfected properly?
      • Re-usable extraction loops were observed during the inspection (semi-critical tools), which were not properly cleaned and disinfected between the clients:a. No high-level disinfectant was available for disinfection, as required. b. No test strips to test the concentration of the high-level disinfectant were available. c. No soaking tray for disinfection in high-level disinfectant was available.d. No designated reprocessing sink was available - the only available sink was located in the bathroom. If re-usable semi-critical tools are used, all issues noted above must be addressed. The re-usable extractors were discarded during the inspection.
