    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The tattooing and storage area was in the basement living room and was not maintained in a clean and sanitary condition for a personal service facility.- Dirty used dishes were stored on top of the tattoo bed in the living room.- The tattooing area was not separated from personal living spaces as there was a free roaming dog, and children's toys in the living room.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Client service area, including the space where clean tattoo equipment and supplies were stored in a corner in the basement living room. There was inadequate separation from personal living areas.
