    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • It was discovered that the operator is providing additional services including invasive procedures (permanent makeup) without notifying AHS. The initial PSN submitted was for haircuts, manicures and pedicures.-Submit a new PSN and include all the services that will be provided. In the meantime, remove the bed from the room and do not provide any services other than haircuts and manicures and pedicures.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Used single-use gloves were stored in a basket among tubes of hair dye. Do not reuse gloves - discard after they are removed.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Disposable used files that were stored among clean ones were observed during the inspection.- Do not re-use any disposable single use items.
    • 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
      • The laundry basket was observed to be overflowing with dirty capes. - Have a large enough basket to store the dirty capes in a way that they are contained inside and not touching the floor or other surfaces.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1) The facility counters had dust build up. 2) The haircutting chairs had hair buildup.- Clean the counters and chairs.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • 1) Staff were unsure of the contents of an old chemical spray bottle they were using to clean and disinfect surfaces. Spray bottle was labelled as Oxivir Tb that had expired in 2023. Only use an approved disinfectant and ensure chemical bottles are labelled appropriately to indicate their content/expiry date.2) Personal food was stored on a cluttered work counter. - Do not store personal food or other personal items on the work counters. Organize work surfaces and have a designated area for personal items.3) From conversation with owner, back room and public space were used for private/personal use (owner stated family members and friends of family). Private/personal implements used in invasive services were stored among public implements in immediate public space. Immediately cease all unapproved personal services and remove all items not related to hair and nails from premises.
