      • * Cockroach was found on a sticky trap under the dishwashing area. Site has a professional pest control company contracted for issue. Continuous treatments/follow ups were being conducted. Last treatment for cockroaches was March 31 with follow up in 3 weeks. Please send PHI the follow up pest control records for review.

    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • *Mouse droppings were found on unused equipment in the back dry storage area, which was notably cluttered. Thoroughly clean and organize the space, removing any items not essential to kitchen operations. Implement appropriate pest control measures to address and eliminate the presence of mice. Please contact professional pest control company. *Cockroaches were observed on sticky traps placed throughout the kitchen. The manager confirmed awareness of the issue and stated that a licensed pest control company is actively managing the treatment process. Manager stated a treatment was applied last night, and pest control company advised there would be increased pest activity is expected as part of the treatment’s effectiveness.Holes were drilled into the wall behind the dishwasher/hallway for cockroach treatment. *Provide PHI copies of the licensed pest control records. Continue to treat for cockroaches/mice and other pest until they are eradicated. Continue to improve the cleanliness of the building and all food areas as it will assist in pest control. Continue to implement all the professional pest control company recommendations.Nov 25/25- Update from management: Mouse droppings on unused equipment in the back of dry storage area was cleaned and sanitized. The spaced was organized and decluttered. Mouse traps are in place. Pest control records were not provided to PHI for review. *Provide PHI copies of the licensed pest control records for review.

      • *Multiple food items were stored directly on the floor and under shelving in the walk in cooler and walk in freezer. Reorganize areas, elevate food products off the floor to protect from contamination, prevent pest access and facilitate cleaning.

      • *Eggs were found stored directly on top of ready to eat sandwich meats in the prep cooler. Reorganize and ensure raw items are stored below ready to eat food items. Eggs were relocated to below sandwich meats. CORRECTED

      • *Staff personal food items were found in the walk in cooler. Discussed with manager, if staff wants to store personal food items in the cooler, a designed and labeled area is required. Items were removed and placed into a labelled container into the walk in cooler.

      • *One ceiling tile located outside the walk-in cooler and freeze was an acoustic ceiling tile. Acoustic ceiling tile was not smooth or easy to clean. Please replace with smooth and easy to clean ceiling tile.

Implement pest control measures to eliminate mice. Provide this office the last 3 months pest control records for review. No mouse droppings found in other areas in the kitchen.

      • *Food container was observed to be in disrepair. Ensure food safe containers are used at all times (containers from sour cream and other food ingredients were observed to be used as re-usable containers). This item was corrected during inspection.

      • *Overall sanitation needs to be improved. Areas were discussed with the operator onsite (i.e. decluttering and cleaning).
