      • Surfaces throughout the facility are not maintained in a visibly clean state. Dust, debris, and residue was observed on surfaces throughout the facility including at pedicure station drawers/storage units (exterior/interior surfaces), manicure stations (exterior/interior surfaces), and storage areas (shelves, cupboards, etc.) for products.Corrective Action: Clean and maintain all facility surfaces in a visibly clean condition including floors, drawers, storage units, shelves, cupboards, and service stations.

      • Facility-specific written procedures were not established for the following:- Cleansing of skin and mucous membranes.- Client care following an accidental skin cut or puncture.Corrective Action:Provided the Owner/Manager with AHS Written Procedure Templates and reviewed the procedures with them.

      • The facility was using expired CS20 (expired as of Sep. 5, 2024). There was no other high-level disinfectant available for disinfecting semi-critical equipment. Facility did not have test strips for high-level disinfectant or a record log for the usage/testing of the high-level disinfectant.Owner/Manager stated the expired disinfectant was used on all semi-critical tools currently in use.Corrective Action:- Advised Owner/Manager to inform staff to stop providing services requiring semi-critical tools (i.e. manicure/pedicure) until properly disinfected tools are available.- Owner/Manager contacted staff member to deliver new high-level disinfectant and test strips during the inspection.- Owner/Manager and staff conducted cleaning and disinfection of semi-critical tools during the inspection.- Provided and completed test/usage log with the Owner/manager.Moving forward, please ensure a high-level disinfectant (i.e. PREempt CS20) is available for disinfecting semi-critical equipment. Furthermore, ensure compatible test strips are available and used to test the high-level disinfectant. A record log for the high-level disinfectant usage and testing results must be maintained.

      • Products transferred from bulk supplies into secondary containers were not properly labelled.Corrective Action:Ensure all secondary containers are clearly labelled with the product name and any other required information.

      • Wax container was noted being in unsanitary condition. spillage of wax all over the exterior of the warmer was observed. There were no lids on the wax containers.Clean the exterior of the wax warmer and ensure the wax containers are covered to prevent the contamination of the contents.

      • Drill bits at the nails station were reportedly being immersed in alcohol and not being disinfected with high level disinfectant.Ensure the drill bits are properly cleaned and disinfected with high level disinfectant.

      • 1-High level disinfectant (HLD) was reportedly not being used according to the manufacturer instructions for use.2-High level disinfectant (HLD) was not being tested prior to reuse every day.3-No HLD daily testing record was available for review.4-HLD test strips were expired.Obtain HLD test strips with valid expiry date. Test HLD prior to reuse daily and enter the test results in daily HLD Testing log form provided during inspection.Follow manufacturer's instructions for use for contact time for each disinfectant.

Reprocessing of semi-critical tools were performed in two hand washing stations located two corners in main service room.The room in the wax room was advised to be designated for reprocessing and install storage and sufficient shelf space for reprocessing of tools and air drying.
