      • 1-Room 3, hand hygiene station was not supplied with paper towels. Operator provided paper towels at hand sink during inspection onsite.2-Hand sanitizer with out NPN/DIN was noted onsite. Operator provided hand sanitizer with NPN and valid expiry date during insepection onsite.

      • Xylocaine with 3 days expiry was noted in the drawer passed its expiration date. The bottle was discarded during inspection onsite. Operator was advised to label the bottle once open to track and discard after 3 days expiry date.

      • 1-Manufacturer's instructions for use (MIFU) was not being followed for neuromodulator use. Both Botox and Dysport requires reconstitution with preservative-free saline, and most be used within 24 hours post reconstitution.Obtain preservative free saline solution and follow Manufacturer's instructions for use (MIFU).-Bottles containing antiseptic products were not labelled.Label all products containing container/bottles to indicate their contents correctly to allow their safe use.
