      • 1. The concentration of bleach/chlorine sanitizer solution in a spray bottle was too strong. The solution bleached out test strips. The PHI prepared 100ppm solution during inspection and instructed the staff to ensure the required 100ppn is maintained at all times.Please use the chlorine test strips at the facility to verify your bleach sanitizer concentration. 2. The facility uses a degreaser solution to sanitizer food contact surfaces at the cooking line. The staff were advised to use 100ppm chlorine solution available at the facility to sanitizer food contact surfaces. Please note that the degreaser solution observed is not an approved food sanitizer.

      • Single use cups were being used as scoops and stored in bulk-food items. Use scoops with a handle and store in a way that protects food from contamination.

      • Previous Violation Some Items not yet Completed:Remediation of walk-in cooler has been initiated. remaining wall and ceiling surfaces located in the walk-in cooler were rusty and deteriorated and required repair/re-surfacing. The interior of the door in this unit was severely dented and required repair2 particle board shelfs were damaged in the dishwashing room and required replacement.
