      • a) Semi-critical items were disinfected soaking in a bottle containing CS 20, and disinfectant does not come in contact with the entire surface of the tool.b) Disinfectant is reused for some days, but no test strips were available to verify concentration of the disinfectant.c) No disinfection log was maintained.Corrective action:a) Soak tools in a tray so that tools are fully immersed in the disinfectant.b) If keeping the disinfectant for more than 1 day, please also obtain test strips for monitoring the strength of the disinfectant and maintain a log book.-Please label the soak tray to identify its contents and contact time.c) Maintain disinfection log.
