- **-- Evidence of mice activities such as mice droppings and chew marks were observed in the facility warehouse**--Wall openings were also observed in the warehouse and storage room in the grocery storePlease ensure that all holes are covered to restrict pest access and harborage in the facility. Also, clean all areas contaminated with mice droppings and sanitize with bleach solution to prevent contamination and reduce tracing by the mice. Traps that are no longer viable should be replaced and food items should continue to be moved away from the wall and floor as much as possible.

- **-- Some shelves in the grocery store were observed to contain unfinished/ unsealed wood. **-- Some baseboards were observed missing in the storage room (top floor) of the grocery storePlease ensure that all wood used in food processing and storage areas is sealed (painted or polished) to make the surface durable, non-absorbent and easilycleanable.Also, ensure that missing baseboards are replaced to prevent harborage of pest such as mice.

- A can of pinto beans is observed with a deep dent in it. Any damaged cans with dents or bulges can have the potential to compromise the safety of the food product by introducing foreign contamination. The operator discarded the indicated can during the inspection.
- 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
- Two QUAT sanitizer spray bottles in the front food prep area are measured at 50 ppm with QUAT sanitizer test strips. Ensure that QUAT sanitizer is maintained at 200 ppm at all times. The operator prepared a 200 ppm QUAT sanitizer during the inspection.
