      • - The glass of the door and window in OSC observed to be cracked. Maintain the facility in a safe and suitable condition by promptly repairing or replacing damaged building components and monitoring for future deterioration.

      • - Chemicals were observed to be stored under hand sinks in the baby/toddler room, pre-school room and OSC room with locks/secured. Chemicals must be used and stored with due care in a manner that prevents hazard or harm to the children. Ensure all chemicals are out of reach of children or are secured by a lock.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • - Client cubbies in pre-school room were observed to unsecured. Ensure all cubbies are fully secured to the walls to prevent hazards and injuries.- Reused pinecones and small pom-poms were observed in the toddler room. Pinecones are single-use natural materials and should not be retained for repeated use. Small pom-poms may present a choking hazard. Ensure all materials accessible to toddlers are safe, age-appropriate, and maintained in a sanitary condition.

      • - Diapering pads in baby and toddler rooms were observed to be ripped. Equipment must be maintained in good working order and in a clean and sanitary condition. Operator replaced diapering pads during the inspection to ensure that equipment remains easy to clean and sanitize.

      • - The walls in OSC accessible bathroom were visibly unclean. Clean and maintain washroom surfaces in a sanitary manner.

      • - First aid kits in baby and toddler room, and preschool room were observed to be not properly supplied/updated or to have expired elements. Recommend to have one Type 2 First Aid Kit in center. Review/replenish contents of first aid kits on a regular basis.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - The glass of the door and window in OSC observed to be cracked. Maintain the facility in a safe and suitable condition by promptly repairing or replacing damaged building components and monitoring for future deterioration.

      • - No pest control records observed on-site. Ensure that a monthly pest control checklist is completed monthly for the facility.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • - Fan in baby/toddler bathroom was not in operation. Repair or replace the fan and ensure it is maintained in good working condition. - The toilet seat in the standard bathroom in the OSC room was broken/in disrepair. Replace the toilet seat and maintain fixtures in good repair. - Peeling paint was observed on the wall in front of the hand sink in the older preschool room. Repair and refinish the wall surface to ensure it is smooth, washable and maintained in a good condition.

Please ensure refrigerators are maintained at 4C or less.Sept 25, 2025 - The fridge in the Toddler Room was at 3C but the fridge in the Baby Room was still at 10C. There was no food stored in the Baby Room fridge

      • -The lego table in the OSC Room was visibly unclean. Please clean.

      • -The sand table in the Older Pre-school Room did not have a lid. Please ensure the table has a lid and it is covered when not in use.

      • -Sinks in the Older Pre-school Room and OSC did not have hot water. Please ensure both hot and cold water are available at sinks

      • -The fridge in the in the Baby Room was at 10C and the one in the Toddler was at 9C. Please ensure refrigerators are maintained at 4C or less.

      • - There were exit doors in the Pre-school and OSC Room that lead directly outdoors. Please ensure that there are safety measures put in place to prevent children from exiting through the doors unannounced.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -Please keep the toilet brush out of children’s reach-There were missing electrical outlet plug covers in the Pre-school Room. Ensure protective covers are in place.- There were several staples in the wall that were not attached to anything. Please remove all unused staples. Both staples and tacks are not recommended; other options are tape, sticky tac, Velcro etc. Staples could also be covered with tape.-Please ensure the door to the storage room in the OSC is kept locked.
    • 05. Are communicable diseases at this facility being properly managed?
      • -Illnesses were not being recorded when children stay home ill. Ensure illness is recorded for both children and staff who are sent home and those who stay home. Outbreaks need to be reported for any cases of illness, not just when there are children/staff who were ill on-site.
    • 08. Is this facility properly constructed (Non-Critical)?
      • - The Pre-school Room had peeling baseboards and lifting floor covering. There were also peeling baseboards in the Toddler Room. Please address.- There was tape on the Toddler diaper table that was peeling. Please address.- Caulking was in disrepair and the wall behind the sink in the Toddler Room was peeling. Please address.- There was a stained ceiling tile in the Older Pre-school Room. Please replace.

      • -Washroom ceiling fans were visibly unclean. Please clean.

      • - Re-useable dishes were being used. This facility has not been approved to serve food and therefore cannot wash dishes. Please remove all dishes.
