      • *Kitchen door was left open while kids were in the hallway waiting to go outside and they were wondering at the kitchen door. -Please make sure kitchen door is kept closed at all times.

      • *Hand washing sink in the explorer's room for the staff - was not stocked with hand soap and paper towels. Corrected during the inspection. Please make sure hand washing stations are stocked adequately.

      • ***Disinfectant use onsite - 100-200 ppm Bleach water solution for table mat children use Cleaning and sanitizing of the table mats used for the children during the mealtimes. Contact time was not properly applied.Proper Cleaning and Sanitizing of Table Mats:_Remove debris (Scrape or wipe off food residue and visible dirt from the table mat)_Wash /clean (wash the table mat with warm water and detergent, use scrub to remove the grease and food debris, rinse with clean water)-Sanitize (Apply an approved sanitizer to the table mat, ensure the sanitizer covers the entire surface)-Contact time (Allow the sanitizer to remain on the surface for required contact time) at least 1 min.

Storing sanitized toys on the floor may result in re-contamination.---Ensure that cleaned and sanitized toys are stored off the floor on clean, non-porous, elevated surfaces (e.g., shelving or bins) to prevent contamination.

      • Observed the diaper changing during the inspection in toddler room.* Safety belt was not put on a child during the diaper changing. * Unproper glove use and diaper changing procedure during the diaper changing. * On of the diaper changing area was posing safety risk. Please make sure diaper changing tables area are rearranged to prevent any injuries and accidents. Please make sure diaper changing surfaces are moved to a location directly access to hand washing. Discussed these concerns with the operator during the inspection.

      • *Outstanding on 13- Aug-2024Toddler Room, Discovery, Little Explorers 1. No hot water at the hand washing station.*All the handwashing stations must have access to hot and cold water.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • *Outstanding on 13-Aug-2024- Toddler Room and Little ExplorersHot water was turned on in the washrooms, but the temperature of the water was measured as 22 degrees Celsius and 26 after shutting the cold water tap off.*Onsite staff and operator was notified and requested to correct the deficiency asap.

      • Stairs by the East Exit:Slides on the stairs was not secured, hazard of entanglement, with an inappropriate fall surface, inadequate landing zone and at the end of slide there was an unsecured cardboard box (picture taken).***Please remove the slide from the stairs immediately.Toddler Room - There was no lock on the cupboards, and scissors were stored in there.***Please keep sharp objects away from the children and install lock on the cupboards.Infant Room *Sanitizer bottle was not labelled. Please make sure it is labelled for the proper handling of the chemicals.

      • 1. Infant Room . Sink in the room by the diapering station is used to wash wiping clothes and staff is washing hands in the washroom.***Please wash hands by the diapering station or move diapering station in front of the washroom, to observe infant’s while washing their hands.2. Milk bottles get sanitized daily, but the water bottles are being sanitized weekly. ***Please make sure water bottles gets washed with proper dishwashing method. They all have to go to dishwasher or manual dishwashing method- wash rinse and sanitize in food grade sanitizer- submerged into the sanitizer with a proper contact time.Little explorers room :*Dust accumulation on the diapering station.

      • Lights cover was broken, and the ceiling was in disrepair.***Please correct the above violation.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Toddler Room, Discovery, Little Explorers 1. No toilet paper and paper towel in the washroom 2. No hot water and soap dispenser was empty at the hand washing station 3. No hot water (hot water was turned off)It was turned on during the inspection was still the water wasn't heating up. ***Staff was standing beside the washroom to provide the supplies.***Children need to have access to toilet paper and hand towel at all times while using the bathroom.

      • Infant Room Wiping towels used for children were stacked on top of each other***Please keep wiping clothes separate, to avoid any cross contamination and change them at each mealtime.

      • Toddler Room :1. Exercise/ Structure Foam Corners are ripped and now it’s not easily cleanable and non-absorbent.***Please repair it or discard.Toddler Room2.1 Freezer was closed with the tape, to avoid children opening and closing it. Tape left the greasy mark and some residue on the corner of the fridge.*** Please attach a padlock to the handles of the refrigerator door, to make it child proof, instead of tape to avoid any greasy stain on the refrigerator and it can stick to hands and spread the contamination.2.2 Diapering station drawer was in disrepair and can cause injury to the children.2.3 Two of the light covers were secured with tape. Please make sure light covers are properly secured. ***Please correct the above violation.

      • Stuffies in use of the children are not washed weekly.***Please wash them weekly or put it away if infants are not using it.

      • Toddler Room and Little ExplorersHot water was turned off in the washrooms of and only cold running water was available for children to wash their hands.***PHI turned the hot water valve on, temperature of the water didn't change.***Onsite staff and operator was notified and requested to correct the deficiency asap.

      • Toddler room 1- One of the staff was observed as not practicing hand hygiene properly. ***It was observed during inspection, Staff was wearing gloves, took the diaper off from one child and discard the diaper in the garbage and helped another kid with the blanket with the same gloves. Later, staff proceed to help the first child and discarded the gloves, didn’t do the hand hygiene, and went on to pick another kid and started playing with them.3. During the inspection in the Toddler Room, one of the staff didn't practice the hand hygiene after changing the diaper. She did change the multiple tasks and no hand washing was done. ***Please make sure hand washing practice is revived with the staff, wash hands properly after diapering and while helping different children as well. Hand washing concern noted during the inspection was discussed with the operator and room lead onsite.

      • Staff was spreading butter on bread while placing the bread on the counter/ table.***Please make sure food handling is done properly. Food Handling must be done in clean surface away from the handwashing sink and on a clean tray.

      • Some of the chemical bottles are stored at the bottom cabinets in the rooms which did not have lock on them.*Corrected during the inspection. They were removed to upper shelves and labelled on them to prevent any injuries. Please make sure all chemicals are managed in safely manner and not accessible to kids.
    • 06. Are proper protocols being followed in regards to client care?
      • In Infant room- Tray washing, bottle wash and hand washing after diaper changing was occuring at the same handwashing sink .***Please make sure bottle wash, tray wash and hand washing are done at the different sitation. Corrected during the inspection. Tray wash and bottle wash will be done in kitchen. room sink will be use only for hand washing.

      • Paper towel dispenser in couple rooms were empty. *Corrected during the inspection. Paper tower dispensers were filled up.

      • -Surface of the cabinet under the sink in the Little Explorers Room' wahroom. **Please repair it.-There were some paint chipping happening in rooms. Please repair.

      • Couple pillows were stored together , they were touching each other. *Please make sure all beddings are stored separately and not touching eachother.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • * In two rooms there were pacifiers stored in same containers which were belong to different kids. ****Corrected during the inspection. Please make sure personal belongings are stored separately to prevent the spread of contiguies disease and illnesses.

      • 1. Infant room:No plumbed in hand sink next to diapering station. A mobile sink was being used for children to wash their hands.COMPLETE THE FOLLOWING:1. Replace hand sink. In the meantime, the diapering station will be relocated to the washroom next to the classroom until the new sink is installed.
