- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
- Operator had not notified the Alberta Health Services about the existence of Personal Service business.Discussed with operator the requirement of the submission of the Personal Service Notification.The PSN was submitted during the inspection.

- Operator had not established the written procedures templates, specific for the busienss.Requirement of the facility specific written procedures was discussed with the operator.Templates for procedures.1)Skin cleansing2)Client care in case of accidental cuts and punctures were provided during the inspection.
- 20. Is reusable equipment disinfected properly?
- Facility was not following the manufacturer instructions for the contact time of High -Level Disinfectant.HLD was being used for about a week, however records maintenance and testing was not done.After discussion with operator following corrections were made.1) Facility staff would make sure to follow the manufacturer instructions for contact time2) Operator mentioned that they would be discarding the solution everyday.Note: Facility can use re-use the HLD for 14 days, however routine testing and log maintenance is required.HLD logs and more information for testing could be provided, if required.

- It was observed that container used for soaking the semi-critical tools was not big enough to allow full immersion.Please make sure that tools are fully immersed in High Level Disinfectant to ensure complete disinfection.
- 24. Are waste sharps disposed of properly?
- It was observed that household glass spice jar was being used as sharps container.Please make sure that puncture resistant sharps container be used for discarding sharps.Sharps are considered biohazard and must be handled and disposed of is a manner to avoid injuries and disease transmission.

- Some of the cosmetic products used in the facility were labelled inadequately and ingredients could not be verified by Health Canada websiteOperator confirmed to remove those products from the facility during the inspection.
