      • * Ceiling tile in the ladies' washroom in the OSC room noted to have what appears to be water stains. Please ensure ceiling tiles are maintained in good repair and ensure possible water source is discovered and addressed as necessary. * Two ceiling tiles in the baby room was noted to be missing. This appear to be following to a water incident. It appears that the ceiling tiles were removed, and a process is in place to have them repaired. Ensure all ceiling tiles are in good repair and possible water source is repaired/fixed.

      • Chlorine concentration of the chlorine bleach was measured between 50 ppm and 100 ppm in the Monkey's room. It was addressed during the inspection - concentration was increased to 1000 ppm.

      • 1. Water stains were observed on the ceiling in the Frog 1 room. Investigate and remove the source of water infiltration and replace the affected tiles.2. Paint was peeling off the wall near the window in Panda 1 room. Please repair.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Hand sanitizer observed in Monkey room expired in November 2023. It was removed during the inspection.
