- Several flies were observed throughout the facility. Ensure monthly pest monitoring is continued and increase frequency of cleaning and sanitizing of the facility.

- Food debris and garbage build up was observed under the ice machine and freezers in the back area. Some spillage was observed around the drive thru area. Clean the above-mentioned areas.

- Quaternary ammonium solution was noted to be at 10 ppm. The operator changed the quat solution during the inspection. The concentration was noted to be at 200 ppm. The operator was further advised to change the quat solution frequently.
- 05. Do food handlers refrain from handling food while ill and have acceptable attire and hygiene?
- Food handlers were not washing hands in between tasks. Importance of hand washing was discussed with the operator during the inspection. Please wash hands in between tasks.

- 1. Dirty dishes were noted to be stored on top of food items. Please be advised that food items must be stored in a manner that protects them from contamination. Please do not store dirty dishes on top of food items. 2. Nonfood related, and staff personal items were stored on top of food items. Please designate an area to store staff personal items. Please be advised that all nonfood related items must be stored away from food related items. Please organize the facility.

- Hot water was not observed at the hand washing sink located in the from prep area. Water pressure was not adequate at this sink. Please ensure that adequate water pressure and hot water is available at the hand washing sink.
- 20. Do food handlers at the facility have adequate food safety training?
- No staff at the facility has a valid food safety certificate. The manager stated that they are in the process of renewing their food safety training.Please complete one of the approved food safety training courses and submit completion certificate to our office. This violation was carried forward from the previous inspection report.
