- No paper towel at the designated hand sink. The designated hand sink must be adequately supplied with paper towels to ensure proper hand hygiene is accomplished. Supply the hand sink with paper towels and ensure they are stored in a dispenser to protect it from contamination.

- The castile-soap wash bottle was not labelled. All products must be adequately labeled to identify the contents inside to ensure safe usage. Please label.
