   • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
     • Accelerated hydrogen peroxide disinfectant measured 0ppm. There were no test strips onsite. Staff were not knowledgeable on how to mix or dispense disinfectant. Ensure accelerated hydrogen peroxide is 0.5% and test strips are available. Ensure all staff know how to prepare disinfectant. After speaking with program director facility will switch to a chlorine based disinfectant for ease and this product does not need to be rinsed after use.
