      • Carbon monoxide detector alarm went off indicating CO issue in the building. PHI asked that the facility get a licensed plumber/HVAC technician in to assess the issue and provide report to PHI. Ensure all recommendations from the licensed technician are followed.

      • TODDLER WASHROOM:1. The area where the sink counter meets the wall in the toddler washroom has a gap and is not sealed properly. Ensure this is sealed2. The area where the toilets meet the flooring is not sealed properly, dirt/debris was observed accumulating around bottom of toilet. Ensure this is cleaned and then sealed properly to avoid dirt build up and allows proper cleaning going forward. 3. The material in front of the handwashing sinks is damaged and does not appear impervious to moisture. Ensure this is repaired so it is smooth, cleanable and impervious to moisture.
