      • A staff member was observed using the same meat slicer to cut ready-to-eat meats (turkey, ham, and salami) without cleaning and sanitizing it between uses. Education was provided on proper meat slicer cleaning and preventing cross-contamination. Meat slicers must be cleaned and sanitized between handling different food items.
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • The food handling permit on display expired in September 2025. Please print and post the current valid permit for public view.

    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • Prep table cooler (sandwich assembly line) was not maintaining temperature. External temperature of foods was 10C. Staff noted the issue was identified during their temperature checks & a service technician is expected later today. PHI instructed staff to only take small portions out for the assembly line & discard all temperature-abused product at the end of the day.
