      • Waxing, threading, brow henna & barbering services were offered without notifying this office.If the facility permanently closes, moves locations, adds new services or uses new beauty instruments, please ensure this office is notified by completing a personal service notification form on-line.

      • 1. Written procedures for skin cleansing was required to be established for barbering services.2. Written procedures for skin cleansing was required to be established for threading services.
    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • Written procedures for cleansing skin was updated at the time of inspection due to a change in skin cleanser availability.Ensure written procedures are updated as needed. Personal service workers must know where these written procedures are kept. Written procedures must be presented to a PHI upon request.

      • Barbicide glass cylindrical jars at the hair stations were soiled and/or half empty. Discussed proper cleaning/disinfection requirements.

      • 1. There was an unlabeled (red-colored liquid) at the waxing station. Discard and/or properly label. Ensure the original container contains the following labels: product identity, manufacturer, directions for use and list of ingredients. 2.
