      • Quaternary ammonium sanitizing solution was mixed and stored in spray bottles and sanitizing pails. 1. The sanitizing pail at the front of the restaurant was found to be at 0 ppm using a QT-40 test strip.2. The spray bottle of sanitizing solution was found to be too high of concentration, at over 1000ppm.3. Cleaning cloths that were damp were placed into the pail of sanitizing solution when not in use.The sanitizing solutions were adjusted and verified to be at 200 ppm using a QT-40 test strip. Please ensure that all sanitizing solutions being used in the facility is at adequate concentrations for effective sanitation.

      • The kitchen exhaust system and hoods are overdue for cleaning. Cleaning was due in January 2025.Ensure the exhaust system and hoods are professionally cleaned.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Commercial dishwasher was not functional and required repair. Three- compartment sink method of dishwashing used with Quats. Ensure that test strips are used for measuring sanitizer in the sanitizing sink.May 13, 2025: The commercial dishwasher was still in disrepair and not in use. 3-compartment sink dishwashing method was in use. The staff present advised the PHI that the required part will be ordered, and the dishwasher repaired. Please ensure 200ppm Quat sanitizer solution is used to sanitize washed utensils or equipment in the dishwashing sink.

      • 1. The concentration of a bucket of Quat sanitizer solution was measured to be at 0ppm. Fresh 200ppm solution was prepared during inspection.2. Used wet cleaning cloths were not being stored in a sanitizer solution. Cleaning cloth was observed on the kitchen counter. The staff removed the cleaning cloth and stored a clean cloth in a 200ppm Quat sanitizer solution during inspection.Please use Quat test strips to measure the concentration of prepared Quat sanitizer solution.

      • Access to the handwashing sink in the kitchen was blocked with boxes and a cart. The staff removed the items during inspection.Please ensure hand washing sinks are always accessible to facilitate proper handwashing.
